Hi everyone: First off, thanks for the suggestion and help: after hearing that many other people have a working PCI-DIO-96, and finding out that the board works fine on other motherboards that we have, we decided it would be far more efficient to grab a new $50 motherboard and replace the one we have. After doing so, the card is now initialized correctly, and everything appears to work fine internally and externally. So, I guess that motherboard is going to be left to the "what the...?" bin for now. It might've been more helpful to have a bit more debugging information when MITE suddenly came across a null address, but I did manage to figure it out on my own (luckily) so I'm not sure.
