How fast of acquisition?  If it's not all that intensive, I would think any 
low end data acquisition card would be able to handle that (NI PCI-6023) 
comes to mind.  I know it's not PC104, but it gives you an idea.

You may want to think about using Nudam modules as well (www.adlinktech.com).  
They work very well for lower speed remote type applications, and they're 
relatively inexpensive.
